If you were born
overseas by a U.S. parent(s) and would like to request a
certified copy of Report of Birth (FS-240) and Certification of
Birth (DS-1350), please completely fill out and sign the
following forms:

Request Form
(please download the form and fill
out completely). This form must be notarized.

Authorization Letter
(please download the form and fill out completely) - this
letter must be notarized.

Travel the World Visas Service
Order Form (please download the form and fill
out completely)

Notarized
copy of the applicant's identification (driver's license)

If
requesting for FS-240 (Report of Birth),
please submit a notarized affidavit attesting to its
disposition or submit the ORIGINAL FS-240
You can also use these forms for obtaining a certified copy of
Report of Death, Panama Canal Zone Birth or Death Certificate,
Certificate of Witness of Marriage, Certification of No Record.
The processing time varies depending on the
availability of your records. We strongly advise you
to apply ahead of time and send your request by FedEx or UPS.

where the birth occured.
