overseas by a U.S. parent(s) and would like to request a
certified copy of Report of Birth (FS-240) and Certification of
Birth (DS-1350), please completely fill out and signed the

requesting for FS-240 (Report of Birth),
please submit a notarized affidavit attesting to its
disposition or submit the ORIGINAL FS-240
You can also use these forms for obtaining a certified copy of
Report of Death, Panama Canal Zone Birth or Death Certificate,
Certificate of Witness of Marriage, Certification of No Record.
The processing time varies depending on the
availability of your records. We strongly advise you
to apply ahead of time and send your request by FedEx or UPS.

Birth Certificate vary from state to
state. The average processing
time is between 3 to 7 days and the VitalCheck fee is usually
between $30.00 to $50.00 for 1 certified copy (including
shipping and handling). Additional fees applies
if you order more than one certified copy. The VitalCheck
Agency will directly bill your credit card for the certified
copy and the shipping and handling. Travel the World
Visas service fee will be charged separately.
