Visa Requirements for U.S.

Valid and signed U. S. Passport
(valid at least 6 months beyond the date of entry; with at least
one blank visa page) – photocopy is not acceptable

One Visa Application Form
completed and signed. Application on thermal fax paper is not
accepted. The application form must have original
signature. Both the left and right sides of the
application form must be completed and signed. (MUST BE TYPED
WRITTEN)

Two Passport size photographs

Invitation
letter approved by the Ministry of Foreign Affairs - Invitation
should be sent through Consular Department of the Ministry of
Foreign Affairs of Azerbaijan. If you do not have a

IMPORTANT: For business, education and humanitarian visas:
if an invitation from a receiving party or partner is not sent
through Consular Department of the Ministry of Foreign Affairs,
travelers should submit an invitation received by fax directly
from a receiving party in Azerbaijan and a letter of request
from the employer.
