months beyond the date of entry; with at least one blank visa
page) – photocopy is not acceptable

completed and signed.
Application on thermal fax paper is not accepted. The
application form must have original signature.

World Health Certificate required showing valid vaccination
against Yellow Fever

Signed letter from the applicant addressed to the consulate
explaining exact nature of trip and the duration of stay.
Also, please include your address in Guinea Bissau.
