PROCEDURE FOR PASSPORT ADDITIONAL PAGES
Please follow the procedure accordingly to avoid processing
delay. An incomplete or incorrectly completed application
will delay the issuance of the passport.
Please send the following
documents to Travel the World Visas for processing:
1) Valid
signed U.S. Passport
2) One Application for Additional Visa Pages form (DS-4085) completed and
signed. Must have an Original Signature. NOTE:
Highly recommended by the U.S. Passport Agency that you fill out the
DS-4085 application form on-line, click
here. After filling out the form on-line, please print
the completed form and sign.

New Passport Agency Requirement effective
December 2004
Please fax Travel the World
Visas a copy of the DS-4085 form that you filled out and signed.
Original signed DS-4085 form must be sent to us
along with the other documents by Federal Express or UPS.

3) Signed letter of authorization.

4) Proof of Departure (one of the following if
requesting for a rush processing)
● Copy of the flight itinerary
● Copy of the airline ticket

● Letter
from your company stating intended travel plans

FedEx tracking number. YOUR PACKAGE MUST BE IN OUR
OFFICE BY 8:00 A.M. FOR SAME DAY PROCESSING.

denied by the consulate or the Passport Agency, or if you
withdraw your application that has already been submitted to the
consulate or Passport Agency.
