PROCEDURE FOR CORRECTION OF
PASSPORT INACCURATE DATA
Please follow the procedure accordingly to avoid processing
delay. An incomplete or incorrectly completed application
will delay the issuance of the passport.
Effective
September 26, 2005, as an additional security enhancement,
Passport Services will no longer amend valid passports. Instead,
customers requesting name changes, extension of validity for
limited passports, or correction of a printing error will apply
for replacement passports.
Please send the following
documents to Travel the World Visas for processing:
1) Actual
passport that needs the
change
2)
Two recent original passport size photographs (2” x 2”) on a
white or clear background (click here
for the regulations for acceptable passport-type photograph)
3) One completed and signed passport application form. Must have an Original Signature.

change is within one year after issuance, please fill out form DS

(U.S. Passport Re-Application Form). NOTE:
Highly recommended by the U.S. Passport Agency that you fill out the
DS-5504 application form on-line, click

change is more than one year after issuance, please fill out

DS-82*
(Application
for a U.S. Passport By Mail). NOTE:
Highly recommended by the U.S. Passport Agency that you fill out the

Visas a copy of the DS-5504 OR DS-82 form that you filled out and signed.
Original signed DS-5504 OR DS-82 form must be sent to us along with
the other documents by Federal Express or UPS.

4) Original or certified true copy of the document
showing the correct data (example: Marriage License, Court
Order, Divorce Decree, birth certificate, etc.)
5) Signed letter of authorization.

6) Proof of Departure (one of the following if
requesting for a rush processing)

● Copy of the airline ticket
or
● Letter
from your company stating intended travel plans

Requirements and fees are subject to change without prior
notice. All fees are non-refundable, if your application was
denied by the consulate or the Passport Agency, or if you
withdraw your application that has already been submitted to the
consulate or Passport Agency.
