PROCEDURE FOR PASSPORT NAME CHANGE

Please follow the procedure accordingly to avoid processing
delay. An incomplete or incorrectly completed application
will delay the issuance of the passport.
Effective
September 26, 2005, as an additional security enhancement,
Passport Services will no longer amend valid passports. Instead,
customers requesting name changes, extension of validity for
limited passports, or correction of a printing error will apply
for replacement passports.
Please send the following
documents to Travel the World Visas for processing:
1) Actual
passport that needs the
change
2)
Two recent original passport size photographs (2” x 2”) on a
white or clear background (click here
for the regulations for acceptable passport-type photograph)
3) One completed and signed passport application form. Must have an Original Signature.

a) If the
change is within one year after issuance, please fill out form DS
5504
(U.S. Passport Re-Application Form). IMPORTANT:
U.S. Passport Agency REQUIRES that you fill out the
DS-5504 application form on-line at https://pptform.state.gov/FraudAbuseNotice.aspx?form=DS11
After filling out the form on-line, please print
the completed form and sign. Please fax us a copy at

change is more than one year after issuance, please fill out
form
DS-82
(Application
for a U.S. Passport By Mail). IMPORTANT: U.S. Passport Agency/Department
of State
REQUIRES that you fill out the DS82
application form on-line at https://pptform.state.gov/FraudAbuseNotice.aspx?form=DS11. After you complete the form on-line, please
print AND sign. Please fax us a copy at 530-579-3315 or
e-mail us at info@world-visa.com
4) Original or certified true copy of the document
showing the name change (example: Marriage License, Court
Order, Divorce Decree, etc.)
5) Signed letter of authorization.
Must have an
Original Signature.
6) Proof of Departure (one of the following if
requesting for a rush processing)
● Copy of the flight itinerary
● Copy of the airline ticket
or
● Letter
from your company stating intended travel plans
7) Government fee:

FedEx "First Overnight" AND call us with your UPS or
FedEx tracking number. YOUR PACKAGE MUST BE IN OUR
OFFICE BY 8:00 A.M. FOR SAME DAY PROCESSING.

Requirements and fees are subject to change without prior
notice. All fees are non-refundable, if your application was
denied by the consulate or the Passport Agency, or if you
withdraw your application that has already been submitted to the
consulate or Passport Agency.
Travel the World Visas acts only as an agent, and accepts no

Passport Agency or Embassy in connection with the granting of
visas, nor for any delays, loss of passports occasioned by such
services or the U.S. Mail. Damage compensation is not
available.
